General search, extreme hurtSam

Hi everyone, I'm looking for some hurt Sam (as usual). Gen only please.

I really want, basically, the most brutal stories you've read, extreme injuries, limb loss maybe, surgery, particularly brain surgery but any kind of surgery is fine, just... I don't know, shock me. Make me hide behind my fingers while I read and wince.

Protective frantic Dean is a must, Dean trying to fix injuries or just trying to keep Sam alive until they get to hospital, that kind of thing. Non-con is fine, pre-series is fine, basically anything as long as it's GEN and has epic hurt Sam with big brother Dean. Oh, and no death fic please.
