spnstoryfinders_ljDean has gotten hurt (a broken leg?) and he and Sam settle in a house while Dean recuperates. Sam starts a relationship with the woman next door - I remember that she has a dog.
Dean observes this relationship evolving and decides that this is making Sam happy; Dean decides that he will leave when his leg is better and let Sam have a normal life. Dean leaves a note saying goodbye; Sam goes after Dean. I don't think it was wincest, but my memory is faulty.
