I know some of the details aren't quite what you've stated but it sounds like the 'Life after the Invasion' series. John and Dean are Watchers, Sam is a Seer for TPTB. Dean has two children and Paige is the Whitelighter for one of them. Hannah is the delicate-looking Slayer.
