Dean leaves Sam to give him a normal life

Dean has gotten hurt (a broken leg?) and he and Sam settle in a house while Dean recuperates. Sam starts a relationship with the woman next door - I remember that she has a dog.

Dean observes this relationship evolving and decides that this is making Sam happy; Dean decides that he will leave when his leg is better and let Sam have a normal life. Dean leaves a note saying goodbye; Sam goes after Dean. I don't think it was wincest, but my memory is faulty.

There is a fairly recent story by chemm80 called "Escape Velocity" which fits some of your description, except that it is Sam who is injured and it is het. But Sam does hook up with the woman next door, who has a dog, and Dean drinks a lot and decides Sam would be better off if Dean left and let Sam have a normal life. Is that the one? It's a great read either way! Here's the link:
