Several of paxlux's stories feature both boys smoking (like Blackout Tendencies), but it's not much of a plot point, it's just, uh, in it for the atmosphere? IDEK. *lol*

No, it isn't Wincest & they are related but when Dean was taking Sam out of the burning house he dropped him & Sam got lost. Sam grew up in foster care & they meet in high school.
